Electronic Statement Access Agreement

PLEASE READ THIS AGREEMENT COMPLETELY.

TRANSMITTAL OF ELECTRONIC STATEMENTS By electing to receive your statements electronically in the manner set forth in the e-statement Procedures (the "Procedures") in question and answer format, you agree to accept your Hoosier Hills Credit Union periodic statements, and all disclosures related to your statements, electronically by a visual text to be displayed on a personal computer monitor. You request that the credit union transmit your statement to you electronically, and you agree to the terms and conditions set forth in this Agreement. SECURITY PROCEDURES You acknowledge that you will be able to retrieve your statements electronically with a personal identification number (PIN), which you will choose. You acknowledge that, if you disclose your PIN to anyone else or if your PIN is lost or stolen, third parties may be able to access your credit union statements. You agree that the credit union will not be liable for any loss caused by the authorized or unauthorized use of your PIN by any third party to access your statements. You agree to keep your PIN in a place of safekeeping, and you agree that the security of your PIN will be your responsibility at all times. You agree to the security methods and techniques set out in the Procedures. You agree that the credit union will not be liable for any loss arising out of your use of e-statement if the credit union observes the security methods presently set out in the Procedures, or as amended from time to time. Further, you agree that the credit union will not be liable for any loss arising out of your use of e-statement under any circumstances if you fail to observe the Procedures and the recommendations set forth in the Procedures. E-STATEMENT PROCEDURES Q. How do I log on?
A. Click here. This will take you to the login screen. Enter your User ID (Member Number) and Security Code (PIN), which you should have received via the postal service. Q. How do I use e-statement?
A. Once you log on to the system, you can navigate through e-statement by clicking the Statement Date for the month you wish to retrieve. Up to 3 months of online statements are available for viewing. You may also use the Manage Your Account link to modify your PIN or e-mail address. There is also a link to View Reconciliation Form. Q. Do I have to be an Online Banking user to view my statements online via e-statements?
A. No. Q. Will I receive notification when my statement is available?
A. Yes You will receive an e-mail notification that your statement is ready to view if you supplied the correct
e-mail address If you change your e-mail address, you must notify Hoosier Hills Credit Union of the change to continue receiving e-statement notifications. Q. Will my electronic statements look the same as my printed statements did?
A. Your electronic statements will look similar to your printed statements Q. How will I receive the newsletter and other inserts that previously came with my printed statements?
A. Links to the newsletter and other inserts, if applicable, will be sent via e-mail with your statement link. Q. What if I lose or forget my e-statement PIN?
A. Please contact a Call Center Representative at (812) 279-6644 or 1-800-865-2612. Q. How is the integrity of my data maintained using Internet access?
A. E-statements use the latest Internet security technology and techniques to help ensure that your statement information is as secure as possible These techniques include user authentication, data encryption, and the use of secure servers, which are designed to keep out potential intruders. Q. What happens to my e-statement history if I de-enroll?
A. All of your e-statement history will be deleted at the time of de-enrollment. If you wish to print or save to a file your available statements, you must do so before de-enrolling Q. Can I receive a printed statement if my computer is not working or I need one for some reason?
A. Yes, you may request a printed statement by calling (812) 279-6644 or 1-800-865-2612. You may be charged $3.00 for each copy requested. ADDITIONAL PROVISIONS You acknowledge and agree that, by requesting that your statements be delivered electronically, you will not receive paper statements. You acknowledge and agree that, if the credit union in its judgment believes that the transmission of your statements electronically is no longer viable, or that any such transmission may give rise to security or operational concerns, the credit union may cease the electronic delivery of your statements and provide you with paper statements in lieu thereof. You acknowledge and agree that you have read and you understand the Procedures set out, that any e-mail address you provide to the credit union is accurate, that you will promptly notify the credit union of any change in your e-mail address in the manner prescribed in the Procedures, that you have a computer that meets the minimum system requirements set out in the Procedures, and that you have a printer or have access to a printer with the ability to download information in order to keep copies of your statements for your records. TERMS AND CONDITIONS DISCLAIMER OF WARRANTIES E-STATEMENT SERVICES (THE "SERVICES") ARE PROVIDED TO YOU ("YOU OR YOUR") IN PART BY LASERTEC ("COMPANY"). USE OF THE SERVICES IS SOLELY AT YOUR OWN RISK. THE SERVICES ARE PROVIDED ON AN "AS IS"AND "AS AVAILABLE" BASIS. COMPANY EXPRESSLY DISCLAIMS ALL WARRANTIES OF ANY KIND WITH RESPECT TO THE SERVICES, WHETHER EXPRESS OR IMPLIED INCLUDING IMPLIED WARRANTIES OF MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE AND NON-INFRINGEMENT. COMPANY MAKES NO WARRANTY THAT THE SERVICES WILL MEET YOUR REQUIREMENTS, OR WILL BE UNINTERRUPTED, TIMELY, SECURE, CURRENT, ACCURATE, COMPLETE, ERROR-FREE OR VIRUS-FREE, OR THAT THE RESULTS THAT MAY BE OBTAINED BY USE OF THE SERVICES WILL BE ACCURATE OR RELIABLE. YOU UNDERSTAND AND ACKNOWLEDGE THAT YOUR SOLE AND EXCLUSIVE REMEDY WITH RESPECT TO ANY DEFECT IN OR DISSATISFACTION WITH THE SERVICES IS TO CEASE TO USE THE SERVICES LIMITATION OF LIABILITY YOU EXPRESSLY UNDERSTAND AND AGREE THAT COMPANY SHALL NOT BE LIABLE FOR ANY DIRECT, INDIRECT, INCIDENTAL, SPECIAL, CONSEQUENTIAL, OR EXEMPLARY DAMAGES, INCLUDING WITHOUT LIMITATION, DAMAGES FOR LOSS OF PROFITS, GOODWILL, USE, DATA LOSS, OR OTHER LOSSES (EVEN IF COMPANY HAS BEEN ADVISED OF THE POSSIBILITY OF SUCH DAMAGES) RESULTING FROM (i) THE USE OR INABILITY TO USE THE SITE OR THE SERVICES, (ii) THE COST OF PROCUREMENT OF ANY SUBSTITUTE PRODUCTS AND/OR SERVICES RESULTING FROM ANY PRODUCTS, DATA, INFORMATION OR SERVICES OBTAINED OR WHICH YOU WERE UNABLE TO OBTAIN OR TRANSACTIONS AFFECTED OR FAILED TO BE AFFECTED (iii) ANY LINK PROVIDED IN CONNECTION WITH THE SERVICES, OR (iv) ANY MATTER OTHERWISE RELATED TO YOUR USE OF THE SITE OR THE SERVICES. DISCLAIMER OF CONTENT You understand and acknowledge that Company assumes no responsibility to screen or review Content provided as part of the Services, and that Company shall have the right, but not the obligation, in its sole discretion to refuse, monitor, or remove and Content that is available on the Site or through use of the Services. Company expressly disclaims all responsibility or liability to you or any other person or entity for the performance or nonperformance of Content review. YOU ACKNOWLEDGE AND AGREE THAT YOU ASSUME ALL RISK ASSOCIATED WITH THE USE OF ANY AND ALL CONTENT. NO RESALE OF SERVICES You agree to use the Services solely for your own purposes and not to use or exploit any portion of the Site or the Service for commercial purposes not related to your personal use. OWNERSHIP OF CONTENT

You understand and agree that your use of the Services entitles Company to use, monitor and preserve copies of all information and Content and to disclose such if required to so by law or in the good faith belief that such disclosure will promote compliance with legal process, enforcement of these Terms and Conditions, assistance in responding to claims that Content violates the rights of third parties, protection of the personal safety of any person, or protection of the property rights of Company or any other person or entity.
